This presentation will explore the importance of Palliative Care and of thinking ahead and making decisions about your future health care, particularly if there comes a time when you are unable to speak for yourself.
Learn about documenting your wishes, appointing a substitute decision-maker and discussing your values and preferences with family, carers and health professionals. Advanced Care Planning helps ensure that your choices are known, respected and guide future decisions.
This session provides practical information and resources to help you take control of future health care planning and gives you the opportunity to ask questions in a supportive environment.
Also looking at what supports are available locally for Palliative Care.
Registration is free but a gold coin is required to attend each session.
